ACUTE CARE FOR THE ELDERLY TEAM

The Acute Care for the Elderly (ACE) program is overseen by the Medical Director and Program Coordinator who is responsible for coordinating the team, for developing the program and for ensuring seamless communication on the unit. 

The Acute Care for the Elderly Team also includes:

  • nurses specially trained in the day-to-day care of elderly patients and the unique problems and needs that arise when older adults are hospitalized;
  • Pharmacists, Dieticians, Rehabilitation Specialists, Speech and Language Specialists, Home Care Coordinators and Care Managers, who are active participants in the team and involved in patients’ care, as needed;
  • and a support staff that includes attending physicians, nursing support technicians, secretaries, housekeepers and volunteers.
