Greater Living’s Geriatrics program is designed for older patients who are struggling to manage multiple chronic and/or life limiting conditions and need help with activities of daily living (i.e. walking, bathing, eating). They are likely to have stressed caregivers who are in need of additional support.

The goals of Greater Living Geriatrics are to:

  • Offer the best in primary geriatric/palliative care for older patients, providing a “medical home” for all of their health care needs
  • Provide anticipatory guidance for patients so that they can envision their future care needs related to their condition(s) and support for medical decision-making
  • Ensure that patients’ transitions from one care setting to another (home to skilled nursing, hospital to home) are seamless and that care needs and wishes are communicated clearly
  • Demonstrate a strong service commitment, including bedside clinical care seven days per week and calls returned by the end of the business day.
