WHO MAY NEED 
GREATER LIVING'S CARE

The Greater Living practice is designed to provide primary care and palliative consults for older patients with two or more chronic diseases (i.e. congestive heart failure, lung disease, dementia) or a life-limiting illness (i.e. cancer, ALS) who need help with two or more activities of daily living (i.e. walking, bathing, etc.) and a more limited life expectancy. These patients often have difficulty visiting doctors’ offices and reside at home or in long-term care or assisted living facilities and are likely to have caregivers who are stressed by the burdens of caregiving and are in need of support services.
