WHERE DOES SUPPORT OUR ELDERS PROVIDE CARE

Services are provided to patients at their place of residence and includes:

  • Certified Registered Nurse Practitioner who serves as primary care provider or supports the patient’s primary care physician or specialist
  • Home or office based primary care – 24/7 telephone access
  • Visit within two working days for urgent needs
  • Physical and cognitive assessments, assistance with Advance Care Planning, medication management, illness and symptom management and coordination with other in-home services
