Individuals who suffer from serious and chronic illnesses often find themselves adrift in a complex medical system that can be uncoordinated and overly complex. For these individuals – too stable to qualify for hospice care or not yet ready to accept hospice – navigating the health care system can create confusion and additional stress. To ensure that they receive the care and resources they need when they need it, in Fall 2013, Gilchrist Hospice Care created Transitions, a non-medical program designed to provide case management and volunteer support for former hospice patients whose health has stabilized, as well as for seriously-ill individuals who are either not yet eligible for or not yet emotionally ready to accept hospice care.
