Transitions offers clients the services of a case manager to help them navigate the often-complex health care system and access available resources, and volunteer support to help them remain in their homes or other place of residence. The Transitions case manager assesses clients for admission to the program, works with clients and their families to identify goals for care, implements and coordinates an individualized plan of services and follows up with the client and caregiver, usually on a bi-weekly or monthly basis. Volunteers offer companionship or caregiver respite, if needed, and will run errands, or perform other tasks, as needed.
