YOUR TRANSITIONS TEAM
Transitions Case Manager:
The case manager works with clients to assess their appropriateness for admission to the program. Once the client is part of Transitions, the case manager performs an comprehensive assessment of the his or her needs, identifies goals of care, creates and coordinates an individualized plan of services, and follows up with the client/caregiver by phone.
Volunteer Services Administrative Coordinator:
The coordinator is responsible for assigning volunteers, as needed for client care, and follows up with phone calls to reassess needs. Volunteers provide the following services for clients and families: companionship, reading, and assistance with writing where appropriate, respite breaks for caregivers, errands and light housework, including food preparation. Through their regular one-on-one contact with clients, volunteers are also able to anecdotally identify changes in a client’s health and will communicate those changes to the case manager, who can locate any appropriate and necessary resources to help the client.
