Gilchrist Transitions is a non-medical program designed to provide continuity of care for former hospice patients whose conditions have stabilized and who no longer qualify for hospice or to provide needed services for individuals suffering from serious illness who are not yet medically-appropriate for or ready to accept hospice care.

Transitions clients benefit from the services of a case manager who helps them identify goals for care, creates an individualized plan of services and assists them and their families with both Advance Care Planning and accessing resources in the community. In addition to case management, the program provides volunteer support for clients in an effort to help them remain in their homes. This support can include respite breaks for caregivers, light meal preparation, assistance with errands and companionship, among other tasks.
