Transitions volunteers are a vital part of the care offered through the Gilchrist Transitions program. Volunteers may offer respite breaks for caregivers, run errands, or prepare meals, among other tasks. In addition, they will be trained to identify any changes in a client’s medical status, which they will then communicate to the Transitions manager to assist with ensuring that the client is receiving the appropriate and necessary care.
